Although Immunizations are required by the State of Virginia for enrollment, the information/tests required may be in conflict with a student’s religious belief. The Certificate for Religious Exemption form may be used to exempt a student from the requirement of the health record.
Procedures for obtaining Immunizations:
- Immunizations at the Health Center are given by appointment only.
- When the student arrives at the Health Center, please check in at the front desk area.
- The student will see the Allergy/Immunization Nurse for the following vaccinations: Tetanus-Diptheria, Meningococcal Meningitis and the Hepatitis B series/titer.
- The Turberculin Skin Test or PPD Assessment will be handled by the UHC triage nurse. If the student actually receives the Tuberculin Skin Test, they will be required to return to the UHC to have the test results recorded as # of mm. of induration within 48-72 hours.
Procedures for obtaining copies of Medical Records:
The University Health Center upholds confidentiality of health information in the highest regard. The UHC will release medical information about a student only if the department receives a completed, signed Medical Release Form. Careful consideration of documentation and information should be realized in releasing medical files.
- Students may request that their Health Record be sent to another institution or agency. For such cases, only the Health Record will be released after the student has signed the appropriate Medical Records Release form. Students may, if they are not currently in the area, send a similar Medical Records Release form to the UHC with their request.
- Students may wish for UHC to discuss their specific case with a family physician, parent, professor, or other designated individual. In such a situation, the student must sign the appropriate Medical Record Release form giving us their permission to release the information.
- The UHC will release relevant information or records if the University or staff member is served with a subpoena for the release of this information.
- UHC will respond to all routine requests for medical information within five (5) business days after receiving a completed, signed Medical Release Form. If, in the University Health Center’s judgment, a request for the release of information is related to a medical emergency situation, we will respond as promptly as possible.
The student has the right to view his/her medical record(s) and to obtain copies of that record. A student must always view his/her medical record in the presence of a UHC Physician or other health professional staff member.
